How to Refer a Patient

Here are all of the ways you can send a patient to our practice:

• Refer them to sign up directly at www.revolutionmed.com.

• Submit our secure referral web form by clicking here.

• Fax your referral to (304) 245-6029.

• Email your referral to info@revolutionmed.com.

• Submit via PicassoMD app.

In-network plan list: Blue Cross, Aetna, United, Cigna, Sentara, Tricare and Medicare.

 

We will contact the patient within 1 business day of receiving your referral.

 

We send all referring primary care providers a monthly progress note, to ensure safe coordination of care.
